What to Bring With You

Please bring a valid state identification card or driver's license with you.

If you are doing a renewal from another doctor then bring your original recommendation with you.

If you had a recommendation from me in the past then you do not have to bring your recommendation with you because we can find you from your name in our system.

You can pay at the office in cash (we have an ATM machine) or with any major credit card.

If you do not have a a state ID card of driver's license then you can bring one of the following instead along with some photo ID: :

• temporary paper license receipt from the DMV
• copy of your lease agreement with California address
• a recent utility bill in your name. The utility bill must be one of the following. gas, water, power, or electric.
  Please note: We cannot accept paystubs or any other bills such as cable, phone, insurance, or tax documents.
